Arthroscopic Usefulness of the Skier`s Thumb (Acute Complete Metacarpophalangeal Ulnar Collateral Ligament Tears of the Thumb)

Abstract

We used arthroscopic procedure to confirm and reduce Stener lesions that achieves spontaneous ligament healing without open surgical repair. 13 thumbs that were suspected as complete tear of ulnar collateral ligaments of metacarpophalangeal joint on physical examination and radiologic evaluation were examed under regional anesthesia. Out of 13 thumbs, Stener lesions were confirmed in 5 thumbs that were reduced by an arthroscopic technique and 8 thumbs without Stener lesion were treated conservatively. The average follow-up period was 25 months. At last follow-up examination, no patient complained pain or functional limitation by joint contracture or instability. In all cases, pinch power were regained to a mean of 92% of the unaffected side. The only complication, articular cartilage damage by equipment, was settled by removal of free cartilage fragment with arthroscopic technique. Results of this study suggests that arthroscopic procedure for reduction of a Stener lesion allows sound healing of injured collateral ligament and decrease the chance of open surgical repair in acute complete tears of ulnar collateral ligament of the thumb metacarpophalangeal joint.
